1. Just type out your story/experience and bring it in on your next visit OR ask us to write it and then you can proof it next time you are in.

VIDEO TESTIMONIAL / STORY

1. Just take a quick video (1-5 minutes) of you telling your story/experience on your phone
2. Email it to concierge@elevatechiroaz.com or mention you have it done next time in the office and we can help you send it to us
3. Or we can shoot the video quickly next time you are in the office
